HEB will be offering two types of flu vaccine injections at RCS.
Flucelvax quadrivalent flu vaccine (prefilled syringe) for ages 7 to 64 - This flu vaccine is designed to protect against four flu virus strains. This vaccine is antibiotic free, mercury free, and can be used for people allergic to eggs. Cost: $39. This is the recommended vaccine.
For patients 65 years and older, Flublock prefilled syringes will be used. Also mercury-free.
Basic trivalent flu vaccine - This is the traditional flu vaccine designed to protect against three flu virus strains. Cost: $12
Please print the HEB Vaccine Consent Form and the Flu Vaccine Information Statement. Please print both pages of the consent form and fill out Sections 1, 2, and 3 only. Please be sure to initial one of the 2 blanks in Section 3. You are welcome to send the form and payment in advance if you are unable to attend with your child and I will get them from class.
