Tomball HEB Pharmacy staff will be here on Tuesday, October 3rd starting at 7:00AM. The Flu Vaccine Clinic will be held in the Nurse's Office which is located in building A.
HEB will be offering two types of flu vaccine injections at RCS.
Flucelvax quadrivalent flu vaccine (prefilled syringe) for ages 7 to 64 - This flu vaccine is designed to protect against four flu virus strains. This vaccine is antibiotic free, mercury free, and can be used for people allergic to eggs. Cost: $39. This is the recommended vaccine.
For patients 65 years and older, Flublock prefilled syringes will be used. Also mercury-free.
Basic trivalent flu vaccine - This is the traditional flu vaccine designed to protect against three flu virus strains. Cost: $12
Please print the HEB Vaccine Consent Form and the Flu Vaccine Information Statement. Please print both pages of the consent form and fill out Sections 1, 2, and 3 only. Please be sure to initial one of the 2 blanks in Section 3. You are welcome to send the form and payment in advance if you are unable to attend with your child and I will get them from class. Or, you can bring the form the morning of October 3rd. Make checks payable to HEB Pharmacy. Since many insurance companies now cover the flu vaccine, you are also welcome to bring a copy of your medical insurance card (front AND back) and prescription insurance card (front AND back) if there are separate cards. There will be no appointment times and I will have extra forms available.
